2. Make a Payment
Pay securely through our Square checkout links. You’ll receive an immediate confirmation and access to schedule your visit.
3. Book Appointment
Use our secure booking portal to reserve your time. All visits are conducted via Doximity video for HIPAA-compliant care.
4. Complete Intake
Submit your secure intake form before your appointment. Include current medications, symptoms, goals—and for first visits, report vital signs.
How Telehealth Works
All visits are conducted virtually via secure Doximity video. A link to the visit can be sent via text or email – you choose. First-time patients must provide vital signs (blood pressure, heart rate, temperature, weight). You may purchase your own recommended bundle directly from us or any other reputable provider. Report vitals via your intake form or live during your visit.
For ongoing members in the Plus or Elite plans, book follow-ups at least 1 week in advance to allow time for shipping of RPM devices. Monthly review of vitals is included with these plans.
Bring Your Own Device
You can purchase or use your own vital sign equipment. Just be sure to select digital and reliable models:
- Automatic Blood Pressure Monitor (arm cuff preferred)
- Digital Thermometer
- Pulse Oximeter (finger clip)
- Bluetooth Scale (recommended)
