Description: Topiramate, was originally developed to treat epilepsy, was approved by the FDA in 1998. It is used off-label for weight loss; specifically for weight gain associated with anti-psychotic medications. In some cases it is also used with Phentermine. It can be used short- or long-term for weight loss safely.

Avoid: History of Kidney Stones; Pregnant; Breastfeeding; Trying to Conceive; History of Anorexia/Bulimia

Side Effects: Dizziness (<1%); Paresthesia (5%); Anxiety (1.1%); Headache (1.7%)

How It Works: Topiramate alters a variety of GABA and Glutamate channels in the brain to suppress cravings and appetite.

Data: Topiramate has independently shown results in meta-analysis of 5.34kg weight loss (95% CI 4.56-6.12kg). In conjunction with Phentermine, the CONQUER and SEQUEL Trials Showed as high as 10.7% weight loss in low-dose and 14% in high-dose groups maintained over 2 years.

Description: Bupropion is FDA approved as a medication for anxiety, depression, smoking cessation, and craving suppression. It is prescribed off-label alone or with Naltrexone for weight loss, as the combination pill Contrave.

Other Names: Wellbutrin XL, Wellbutrin SR, Forfivo XL, Bupropion

Avoid: History of Bipolar Disorder; Schizophrenia; Hypertension; Concurrent substance use disorder.

Side Effects: Activation of mania in bipolar patients (<1%); Insomnia (11-40%), Suicidal thinking in 18-24 year-olds with major depression and other psychiatric disorders.

How It Works: Bupropion works by inhibiting neurotransmitter reuptake of norepinephrine and dopamine. By holding higher concentrations of these in the brain, neurons increase signaling and alter hunger and mood.

Data: Bupropion has been studied extensively independently and in conjunction with Naltrexone (Contrave). The COR-I, COR-II, COR-BMOD, and COR-Diabetes trials demonstrated 8.1-11.5% weight loss over 2 years.

Description: Naltrexone was developed as a medication for alcohol and substance use disorder. Although its mechanism is to stop cravings, it has long been prescribed for weight loss. It is a component in the combination pill, Contrave.

Other Name: Naltrexone

Avoid: History of Anorexia/Bulimia; Concurrent substance use disorder; Use of opiates; Pregnant; Breastfeeding; Trying to conceive.

Side Effects: Abdominal Pain (11%); Anxiety (12%); Headache (25%).

How It Works: Naltrexone is a mu-receptor antagonist (blocker). It inhibits the hypothalamic-pituitary-adrenal axis and suppresses appetite and cravings.

Data: Naltrexone has been studied in alone and as a component in Contrave. Studies show that when combined with Bupropion 8.1-11.5% of weight loss was achieved.

Description: Semaglutide is a GLP-1 receptor agonist created for Type 2 Diabetes and FDA approved in 2019. It was approved for the indication of weight loss as Wegovy® in June 2021.

Generic Name: Semaglutide

Contraindicated: History of Thyroid C-cell tumors, thyroid medullary cancer, and multiple endocrine neoplasia; Pregnant; Breastfeeding; Pregnancy Planning.

Avoid: History of anorexia/bulimia.

Side Effects: Nausea (44%); Vomiting (25%); Diarrhea/GI distress (30%).

Description: Tirzepatide, approved for both Type 2 diabetes and weight loss, works through both GLP-1 and GIP receptors to slow stomach emptying and decrease appetite.

Generic Name: Tirzepatide

Avoid: Pancreatitis Gallstones; Family history of Thyroid Cancer; Thyroid disorders, Type 1 Diabetes, Pregnancy, Chronic malabsorption, Cholestasis/History of Gallbladder disease; Pancreatitis.

Side Effects: Nausea (44%); Vomiting (25%); Diarrhea/GI distress (30%).

How It Works: GLP-1 and GIP receptor agonists decrease the rate of gastric emptying. This makes you feel full. GIP modulates chemical signaling in the brain associated with appetite, and both mechanisms work to decrease cravings.

Data: The SURMOUNT-1 trial showed expected weight loss of 22.5%. Additional trials have shown this medication is twice as effective as Semaglutide 1.0mg.

Generic Name: Phentermine and Topiramate

Avoid: Uncontrolled hypertension, coronary artery disease, arrythmia, heart attack, glaucoma, pregnancy or pregnancy planning, severe liver impairment.

Side Effects: Increased heart rate (70%), insomnia (11%), headache (11%).

How It Works: Appetite suppression and increased satiety via enhancement of GABA(A), suppression of AMPA, and neuronal sodium channel blockade.

Data: Trials have shown that people lose about 11% of their body weight after one year with Qsymia, with low calorie diet and exercise.

Schedule an appointment date that allows for bloodwork if you haven’t had blood drawn within the last year.

Complete HIPAA compliant medical forms (sent via email) detailing family and medical history, current medications, prior weight loss attempts, upload ID and lab results. Obtain blood work prior to your telehealth visit to avoid delays in prescriptions.

Meet with your medical provider. Discuss your intake form, lab results, and all options for medical weight loss. Then, obtain your prescriptions, if appropriate.

Receive your scheduled medication in the mail directly from the compounding pharmacy.
